How We Help
Age Safely at Home
We help families determine whether their loved one can continue living safely at home by looking at daily activities, mobility, home safety, caregiver support, and overall care needs. We identify practical ways to reduce risks—including adaptive equipment, home modifications, and additional support—and create a phased plan for aging in place when appropriate. When needs suddenly change or a crisis occurs, we help families quickly understand their options, coordinate next steps, and move forward with clear, compassionate guidance.
Navigate Care Transitions
When a hospitalization, discharge, or sudden change creates urgency, we help families coordinate the next steps for a safe and smooth transition. We communicate with hospitals, skilled nursing facilities, caregivers, and senior living communities; assist with discharge planning; identify potential safety concerns; and help coordinate the services and support needed for continuity of care. Throughout the transition, we keep families informed and provide clear, compassionate guidance so they don’t have to navigate complex care decisions alone.

Explore Senior Living
We help families understand their senior living options, including independent living, assisted living, memory care, respite, and skilled nursing. We carefully vet communities and provide curated recommendations based on your loved one’s care needs, preferences, location, and budget. From pre-tour preparation and scheduling to accompanied in-person or virtual tours, side-by-side comparisons, and post-tour debriefs, we help you evaluate each option with confidence. Once a decision is made, we coordinate admission details and move-in readiness to help make the transition as smooth as possible.
Real Estate and Transition Support
When a transition means leaving a longtime home, we help families navigate the real estate decisions that come with it. Our support can include assessing the property, considering timing and pricing, and coordinating strategic repairs or improvements through trusted vendors when appropriate. For families who choose to work with us, licensed real estate services are available in-house—from preparing and listing the home to negotiations and closing—whether selling as-is or making improvements to help maximize its value. This service is completely optional, and families are always free to choose their own real estate agent.

Every family’s situation is different, so we begin by listening. We take time to understand what has changed, what feels most urgent, your concerns, and what your family hopes to accomplish. We answer your initial questions, help bring clarity to the situation, and explain how Compass Senior Living Advisors can support you as you determine what comes next.
2. We Assess the Full Picture
Through our Compass Care Clarity Assessment, we take a closer look at your loved one’s current situation—including daily activities, mobility, safety, care needs, living environment, caregiver support, preferences, location, and budget. This helps us look beyond the immediate problem and understand the whole picture before recommending the next step.
4. We Walk With You Through the Transition
Once your family decides on the next step, we help turn the plan into action. Depending on your needs, that may include coordinating tours, communicating with communities and care partners, supporting admission and move-in readiness, helping navigate discharge or transition details, or connecting you with additional resources. Our role doesn’t end with a recommendation—we stay engaged through the transition so your family has guidance along the way.
3. We Build Your Personalized Path Forward
We help you understand the realistic options available based on what we learn. That may mean strengthening support at home, planning for aging in place, coordinating care after a hospitalization or sudden change, or exploring senior living. When senior living is appropriate, we carefully vet and match communities, compare options, prepare you for tours, and help your family make an informed decision—not simply hand you a list.
About BreJeanna
I’m BreJeanna Webb, founder of Compass Senior Living Advisors. As a Certified Occupational Therapy Assistant and Certified Dementia Practitioner with more than 15 years of experience, I’ve helped older adults and families navigate changing care needs, safety concerns, and difficult transitions.
My understanding of this journey is personal, too. I walked alongside my mother through her health and aging journey until her passing in 2016, and my family continues to navigate the realities of caring for my aging father and my brother with special needs. These experiences have given me a firsthand understanding of the decisions, emotions, and responsibilities families carry when someone they love needs more support.
I created CSLA so families don’t have to figure it all out alone. My goal is to listen first, bring clarity to the options, and help families confidently take the next right step.
As a licensed Realtor, I can also assist when selling a home becomes part of the transition. Real estate services are completely optional, and families are always free to choose their own agent.

My vision is a connected path of care that helps older adults live with dignity and helps families move forward with confidence—whether the right next step is safer living at home, more support from caregivers, or a new senior living community.
